Responsibilities
- Coordinate comprehensive mental health counseling services tailored to individual patient needs using trauma-informed care principles.
- Manage patient case files efficiently, ensuring accuracy and confidentiality within electronic health records (EHR) systems.
- Advocate for patients within the healthcare system and community resources to support recovery and resilience.
- Collaborate regularly with multidisciplinary teams including psychiatrists, social workers, and community partners to develop and implement care plans.
- Lead crisis intervention efforts, assessing risk and providing timely support in emergency mental health situations.
- Facilitate and support mental health program development aligned with organizational goals and community demands.
Qualifications
- Bachelor’s or Master’s degree in Psychology, Social Work, Counseling, or related mental health discipline.
- Minimum of 3 years’ experience in mental health counseling or case management within healthcare settings.
- Demonstrated knowledge of DSM-5 diagnostic criteria and experience with electronic health records (EHR) documentation.
- Preferred certification in trauma-informed care or crisis intervention training.
- Strong communication and interpersonal skills enabling effective collaboration across diverse teams and with patients.
