Key Responsibilities
- Coordinate and manage patient care plans to ensure seamless, patient-centered service delivery across multiple healthcare providers and settings.

Essential Qualifications
- Current licensure as a healthcare professional in Canada, such as Registered Nurse (RN), Registered Social Worker (RSW), or other relevant regulated health profession.
- Demonstrated experience or strong interest in case management, patient advocacy, and care coordination.
- Excellent verbal and written communication skills to engage effectively with patients, families, and healthcare teams.
